Adoption Online Application Form

Submission of the application form does not commit you to adopt a pet, but provides information CARE needs to begin the adoption process. You will be contacted by a CARE volunteer caseworker to discuss your application and advise what is the pet's adoption status.

Prior to completing application, you may wish to review the CARE Adoption Contract.  CARE requires an adoption fee of $150 to adopt a dog or puppy ($250 for a dog in the Paws in Prison program) and $100 to adopt a cat or kitten; these fees are used to help rescue other animals.

If you prefer, you may print the CARE Adoption Application and hand-deliver it to CARE headquarters, 5516 Kavanaugh Blvd., Little Rock (Monday-Saturday, 10AM-4PM), or mail  the form to CARE, P.O. Box 7604, Little Rock, AR 72217-7604, or scan/email the form to care@careforanimals.org.
